To save you time a GMS1 form can be downloaded and printed if you visit www.gov.uk, however you will still need to complete a new patient questionnaire. Both of these forms are available at reception. You will need to provide two forms of ID, a photograph and a utility bill, not more than three months old (a copy will be suitable), to confirm you live in the Practice area. Please allow 24 hours for us to register you before making appointments.
Non-UK citizens registering with the surgery are advised that doing so may not necessarily entitle them to free access to other NHS services, (e.g. hospital treatment) and they are advised to clarify their individual situation with the Cheshire Clinical Commissioning Group (CCCG) Tel: 01625 663477.
Any changes to your registration details must be notified to the practice.
If your new address is outside of the practice area you will be asked to register with a practice in your new locality. It sometimes comes as a surprise for patients who move only a small distance to be asked to do this - if it is important to you, you are advised to check with the practice that your new address is within the practice area. Although the practice boundaries are shown on the map on the Practice Area page, this is only a guide. Our reception staff will be able to confirm whether your new address is within the area if you give them the exact address.
Patients who change address without notifying the practice will be de-registered and their records forwarded to Cheshire Health Agency.
